Step 1. Review/update the Medical

Patient has HTN controlled by medication Norvasc 10mg daily/ hydrochlorithizide 50 mg; opermazole 40 mg for GERD and cetirizine 20mg for allergies dry mouth/Dental History axiUm form for data applicable to the analysis. (see below)

Step 2. Analyze the Nutrients Report, Caries Risk Form and Medical/Dental History for factors needing to be included in the nutrition counseling.

Step 3. a Dental Nutrition Patient Reflection Paper Should address ALL of the following:

*How does the medical/dental history impact the patient’s nutrition?

* How does the patient’s nutrition impact the patient’s dental and total body health?

* How does the patient’s nutrition compare to Daily Food Group targets and average nutrient intake?

* Compare the number of calories consumed with the number of calories needed as determined by the patient’s activity level.

* Discuss/evaluate the amount of protein consumed.

* Discuss/evaluate the amount of carbohydrate consumed-simple, complex and fiber.

* Discuss/evaluate the amount of fat consumed-saturated, polyunsaturated, monounsaturated and cholesterol.

* Discuss/evaluate the intake of: Calcium, Iron, Sodium, Folate and other nutrients significantly deficient/ in excess.

* Determine if there are any adverse nutrient interactions.

* Provide personalized recommendations based on the patient interview and Food Tracker program, Caries Risk findings and Medical/Dental history findings.

Step 4. The student will submit a Dental Nutrition Patient Reflection Paper containing personal patient information, dietary analysis and recommendations to the CLN 8260/8270 course director for review (48 hours prior to seeing the patient for appointment #2), discussion and grading utilizing the Dental Nutrition Patient Reflection Paper Rubric. Modifications will be made as necessary in order to provide applicable nutritional counseling.
