For Problem 2: You MUST select a problem from the end of the chapter “Exercises” from Chapter 9: Problems 39-86.

These problems and their discussions will be included together in one post. After you decide which problems you would like to discuss, start a thread in the Topic Forum. Ideally, you will choose problems that you had a hard time solving until something clicked (which you would then explain–what you figured out that you did wrong, etc.). Either way, you should lead a discussion about the problem. This will help everyone go through the thought process of these problems, see different ways that students set them up, compare approaches, maybe learn new strategies and hints, recognize mistakes, etc. If you cannot solve the problem at first, no worries, your classmates and myself will give you help you along and you can edit your problem until it is due. If you are having trouble, show your work and explain where you are getting stuck. Project Completion: Medication workflow

To determine the medication administration workflow I would need to get a good representation on the current methods. I start by acknowledging my own concerns and the things I liked about the current methods. I would also need an adequate population sample of nurses willing to participate in the project. This could be done by handing out questionnaires to the staff in one unit. Once they were completed and returned I would input the results into a spreadsheet type format. I would then move onto other inpatient units within the hospital. The same questionnaire would be used for the other groups as well. I would also need to observe an adequate sample of nurses giving medications, our usually times for medication administration in the morning is 9:00am. I would need to have people who would be willing to help observe how many times a nurse is interrupted, if scanning medications takes place, if the appropriate equipment was available to them, and if the medication was available.

Our current method of medication administration involves the scanning of medications through the use of our electronic health record, it would be interesting to see what improvements staff nurses would like to see done. From my own experience, I have found that things such as not clicking “accept” and interruptions have a huge impact on medication delivery. Our organization makes sure we have the right amount of equipment to ensure that we can properly scan medications and administer them safely. At times it is difficult to administer medications in a timely manner due to delays in pharmacy or the method the physician orders them.

Presentation of findings to leadership

I would present my findings initially to my unit leadership team. I would present them the information in a meeting format and would need to have proper format. I would think that presenting the information in graph format as well as in the form of a research paper.

In the healthcare field, technology and programs for charting is always changing or upgrading. I have worked with my healthcare facility for almost 3 years now and there is talk that they will be changing our charting to EPIC soon. It is a struggle for some nurses who are not tech savvy to keep up with this modern technology and changes. On average nurses spend roughly around 28-50% of their work day charting on the computer. (McGonigle 130) Unfortunately, I don’t know exactly how medication administration is performed at my work because I am still a nursing assistant. I know my work doesn’t have wrist bands to scan, instead the patients have room numbers and their name outside their door.

First: I would organize a redesign team for my analysis. I will choose managers or charge nurses who are head of medication administration or oversee RN’s performing this task. They will find an issue to compete with to fix.

Second: Create a value-added activity. Have the redesign team add name/DOB stickers for each individual’s medications and also that each individual has a wrist band identifier to scan to clarify. Order sets, multi-parameter alerting, nursing care plans, reference content, and drug information databases are five elements of the clinical decision support that KLAS Enterprises has recognized. (Versel, 2011) I believe that wrist bands would help my work tremendously. If a patient is in the hallway asking for medication without a proper identification, the nurse must bring the Pt back to their room to clarify the Pt room number and name on the door. I believe that having a wrist band to simply scan on the spot would save a huge amount of time.

What parts of the process could be supported with computers or other technology?

Computers can be used to allow nurses to chart on their medication distribution. Charting will monitor exactly which nurse is administering the medication, what medication is given, and the patient receiving the medication. Scanners connected to the computers will also be used to scan the wrist band on the patient. After the nurse is done charting for the work day, that nurse can be presented with an evaluation of how this process helps or worsens the workflow for medication administration.

How would you document and present your findings to the facility leadership staff?

I would hold a staff meeting for the nurses who have used this charting for medication administration and the redesign team to discuss the evaluation of the charting and the evaluations. Reveal the percentage who thought the charting process helped the workflow and vice versa. I believe a pie chart would show the impact the value-added activity the best.
